It happens for sure when you try to load media to your WordPress Blog Post or bage, or, rather when you try to access the media up-loader from the WordPress editor. The reasons could be inherent to WordPress, since WordPress 2.6 added support for Gears, to speed up the administrative interface and reduce server hits (wikipedia). It could be inherent to the recent Firefox update or to the Windows Vista Service Pack 2, or to Adobe Flash. In as much as I find, it’s not a Vista Issue as it also occurs on XP. For once Microsoft is out of the race. The culprit is the incompatibility between Firefox with WordPress Media loader and Google Gears and Adobe. Na!
